An unfortunate fall, a sudden malaise, a sharp pain, an accident – in all these cases, first aid comes to the rescue. However, doctors are not always able to make accurate diagnoses without the use of sophisticated medical equipment. It is of high importance that it is reliable and works correctly. These qualities are guaranteed by the procurement of medical facilities through life cycle contracts.

The standard procedure for public procurement of medical equipment is as follows: the customer announces a tender, defines its terms and chooses the best supplier. There is a downside: the contract is closed when the money is paid, and the equipment is transferred. At the same time, it is a medical institution that is responsible for equipment maintenance, software updates, and staff training. As a result, there is a need for a new state contract, this time to maintain the equipment in working order.

Life cycle contracts do not have such problems and their advantages are obvious. This form of public-private partnership allows medical organizations to purchase not only equipment but also maintenance services for 7–10 years. The seller undertakes to train medical personnel and engineers, guarantees timely software updates and fixes a breakdown within twenty-four hours.

Ultrasound machines, X-ray equipment, tomographs – all this complex equipment is purchased by Moscow medical institutions since the end of 2019 according to the scheme described above. The interest of health care institutions is clear. However, this scheme is also beneficial for suppliers. These include General Electric, Siemens, and other major global manufacturers. A life cycle contract for them is a guaranteed income over a long period, provided, of course, that the obligations are fulfilled in good faith.

The benefits to the city budget are also evident. Under the terms of the contract, payment for the equipment is made in installments over three years. And the rate is noticeably lower than in commercial banks: the Central Bank key interest rate minus a reduction factor. Moreover, the last is also determined during the competition, when potential suppliers offer not only the lowest price for the equipment, but also installments at a more favorable rate.

But it is patients who benefit most from this form of cooperation. For example, thanks to life cycle contracts many clinics in Moscow were able to buy unique ultrasound machines that can detect diseases even in the early stages because of improved image quality: the "picture" remains clear even when zooming in. As a result, it allows early detection of a disease and its effective treatment.

The contract is paid not for the maintenance of equipment, but for its proper functioning. If the equipment is out of order for more than one day, the monthly payment is not made. For example, if the idle time lasted more than five days, the supplier pays a fine (the quantity of days of idle time is determined by the results of the tender).
